Insurance

As a service to our patients, we will do everything we can to assist you with your insurance needs. These services may include verification of benefits and preauthorization of procedures. We will submit your claim to your insurance company under the benefit coverage you specify. However, please remember that as the patient you are ultimately responsible for payment of the account, including any balances not covered by insurance.

PLEASE NOTE:  We will do pre-authorizations whenever possible.  Those insurance groups marked in blue require that authorizations be done by the referring doctor.

Siker Medical Imaging is an approved provider for most major insurance carriers and health plans including, but not limited to, the following:

*We also contract with most Medicare managed plans, most Workers Compensation plans, and accept all MVA (motor vehicle accident) plans.

**With Prior Authorization we accept Providence EPO, Providence Personal Option, Providence Medicare, Providence Medicaid (Assurance or Providence OHP).

If you do not see your insurance provider listed above, please call us at 503-579-5000.

Siker Medical Imaging will submit your claim directly to the insurance company on your behalf. You will receive a billing statement for any balance not covered by insurance.

For patients without insurance, we offer discounts for payments at the time of service. We accept cash, check or major credit cards. If you need to spread payments over time, our billing department can set up a monthly payment schedule for you. Call 503-595-3967 for further information.
